We team up with you to extend care beyond the office, identifying health risks and providing care that supports recovery during the first three months postpartum. We start with planning during pregnancy for the 4th Trimester. Our ideal care model was developed by an interprofessional team of licensed healthcare professionals, 4th Trimester Care is here to help answer ACOG’s call for Optimizing Postpartum Care.
4th Trimester Care Team Includes Licensed:
- Nurses
- Dietitians (also Lactation Consultants)
- Mental Health Professionals
- Pelvic Floor Physical and Occupational Therapists
The synergistic component is Proactive Health Liaisons (licensed healthcare professionals) bringing continuity of care before delivery and extending through the 4th trimester. After birth, regular check-ins and mental health screenings hold your patients’ hand in navigating the uncertainty.
